I hereby give permission for my child to participate in the Family Over Me (FOM) / JYA Basketball Tryouts. I understand that registering for and participating in the tryouts does not guarantee my child a position on a team or placement on a roster.

Team selections will be determined by the coaching staff based on player evaluations and the needs of the team. I understand and respect that final decisions regarding team selection and roster placement will be made by the coaching staff following the tryouts.

I also understand that the tryout registration fee is non-refundable once payment has been made, regardless of whether my child participates in the tryouts, is unable to attend, or is selected for a team.
