Would you like to help shape the future of our parent-carer forum?
Use this form to put yourself forward for the Steering Group at our 2026 Annual General Meeting (AGM). Current Steering Group members who want to continue should complete it too.
Under our current constitution, candidates must be members of L4C and have previous experience of volunteering with the forum. You do not need professional qualifications.
This is a nomination for the existing forum’s Steering Group. It is not an application for a paid job or an appointment as a trustee of a future CIO.
AGM: Monday 23 November 2026, 12:30–1:30pm, online.
Nominations close Monday 16 November 2026.
Membership is open to parents and carers of a child or young person with SEND aged up to and including 25 who live in, or receive services from, the borough of St Helens. Please do not give your child’s name or medical details.
A few words are enough. For example, helping at events, supporting a workstream or representing the forum. Current SG members can describe their existing involvement.
Tell us a little about what you would bring and what you hope to help with. Plain English is absolutely fine — this does not need to be a formal application.
Your name and this statement may be shared with members for the AGM and any election. Please leave out private information about your family.
Optional. You can simply ask us to contact you — there is no need to share a diagnosis or medical information.
What happens next?
We will contact you about your nomination and the AGM. Steering Group membership runs until the following AGM. Chair, Treasurer and Secretary are chosen separately by the Steering Group after the AGM.
The forum will use your answers to administer nominations and contact you. Your email address and support request are not part of your candidate statement.
