How well are we supporting patients between appointments in cancer care?

This short self-assessment is designed to support reflection, not inspection.
It helps cancer and oncology teams understand how effectively patients and carers are supported between appointments, where anxiety and uncertainty often peak. The questions focus on everyday practice rather than policy and on what patients experience in reality.
There are no right or wrong answers. Honest reflection is more useful than a high score.
