NeuroMetabolic Reset Eligibility Questionnaire

This short eligibility questionnaire checks fit, safety, readiness, location, and budget before you choose a structured NeuroMetabolic Reset package.

It is the start of the package path: answer once, then the next page explains the package direction that makes the most sense. If you are eligible and ready, payment and onboarding come after this fit/safety screen.
