FORM A — PARTICIPANT CONSENT, RISK ACKNOWLEDGMENT AND ADULT RELEASE

1. Parties and event

The service provider is Studio 111 LLC, operating as 4 Knight Wars (Company). This agreement concerns the event identified by its event name, date, location and booking identifier below, including setup-area access, participation, observation and departure from the activity area. A rescheduled event requires confirmation of the new date and a new or reaffirmed consent record. Each adult signs for themselves; each child's parent or legal guardian signs for their own listed children. A host, friend, sibling or another child's parent may not sign merely because they invited or transported a child.

2. Activities and risks

Activities may include mobile foam-dart/Nerf battles, water-gun battles, inflatable target practice, dodgeball, PE-style obstacle activities, foam-sword and pool-noodle games, and use of the equipment selected for the event.

Risks include dart or water impacts to the eyes, face or body; accidental contact with foam equipment; collisions between participants or with obstacles; slips on wet surfaces; trips over uneven ground, equipment or cords; falls; bruises, cuts, sprains, fractures, dental or eye injuries, concussion, heat illness, dehydration and allergic reactions. Severe injury, permanent disability or death can occur. Outdoor conditions, wind, lightning, heat, other people's conduct and equipment failure can increase risk. Safety equipment and supervision reduce but cannot eliminate these risks. I voluntarily accept the inherent risks of the activities I choose, subject to the legal limits stated below.

3. Health, safety and conduct

I will consider my and my child's ability to participate safely, ask questions before participating, and privately notify staff of safety-relevant limitations, allergies or accommodation needs. A diagnosis alone is not a blanket disqualification. I will stop participation if pain, illness, distress or unsafe conditions develop.

Participants must follow the safety briefing, age and equipment restrictions, posted rules and staff directions; keep required protective eyewear/face protection in place; use only approved equipment and projectiles; avoid intentional head/face shots, physical fighting, dangerous contact or equipment modifications; and stay within the designated play area. No participation while impaired by alcohol or drugs. Do not climb, sit on or jump on inflatable barriers or target equipment unless specifically designed and approved for that use. Keep water away from electrical equipment. Report damaged equipment immediately and stop using it. Staff may pause or stop unsafe play. Removal and any refund decision remain subject to the booking terms and applicable law; removal does not automatically forfeit a damage deposit.

4. ADULT'S RELEASE OF ORDINARY NEGLIGENCE

IN EXCHANGE FOR PERMISSION TO PARTICIPATE, I, IN MY INDIVIDUAL CAPACITY, RELEASE STUDIO 111 LLC AND ITS OWNERS, OFFICERS, EMPLOYEES, CONTRACTORS AND VOLUNTEERS ACTING IN CONNECTION WITH THIS EVENT (RELEASED PARTIES) FROM MY OWN CLAIMS FOR PERSONAL INJURY, DEATH OR PROPERTY DAMAGE ARISING FROM THE ACTIVITIES, INCLUDING CLAIMS CAUSED IN WHOLE OR IN PART BY THE ORDINARY NEGLIGENCE OF A RELEASED PARTY, WHETHER SOLE OR CONCURRENT. THIS INCLUDES ORDINARY NEGLIGENCE IN EVENT INSTRUCTION, SUPERVISION, EQUIPMENT INSPECTION OR MAINTENANCE, AND ACTIVITY-AREA SETUP.

THIS RELEASE DOES NOT EXTEND TO GROSS NEGLIGENCE, RECKLESS OR INTENTIONAL MISCONDUCT, OR A CLAIM THAT APPLICABLE LAW DOES NOT PERMIT ME TO RELEASE. IT DOES NOT REPRESENT THAT I CAN EXTINGUISH A MINOR CHILD'S INDEPENDENT RIGHTS. ANY RELEASE OF A CLAIM PERSONALLY BELONGING TO ME AS A PARENT APPLIES ONLY TO THE EXTENT TEXAS LAW ALLOWS AND DOES NOT RELEASE THE CHILD'S CLAIM.

5. Parent/guardian consent

I am the parent or legal guardian of each child listed. I authorize their participation in the identified activities, have explained the rules in a manner they can understand, and will cooperate with reasonable safety instructions. I understand that my consent and risk acknowledgment do not guarantee that a pre-injury release of a child's claims is enforceable. I do not sign for another family, another adult or an unidentified participant. Children and adults not covered by the appropriate consent process may not participate until that process is completed.

6. Emergency response

If reasonably necessary, I authorize staff to call emergency services and provide reasonable assistance within their training. Medical professionals determine appropriate care. Staff will reasonably attempt to contact the parent/guardian or emergency contact without delaying urgent assistance. I am responsible for medical costs for which I am legally responsible; this does not waive any claim excluded from the release.

7. Responsibility for misconduct

To the extent permitted by law, I agree to reimburse the Company for reasonable, documented amounts it is legally required to pay to a third party to the extent caused by my negligent or intentional acts or my material breach of this agreement. This is not an agreement to pay for the Company's negligence, to reimburse the Company merely because my child makes an injury claim, or to waive a child's rights indirectly.

8. Promotional media permission

By affirmatively accepting the media permission below and signing this waiver, I authorize Studio 111 LLC to photograph and record me and my listed children during this event and use those photographs and videos to promote 4 Knight Wars on its website, social media and marketing materials, without compensation. I confirm that I am authorized to grant this permission for each child listed. This permission does not authorize disclosure of medical information, contact details or misleading endorsement claims.

Media permission is required to submit this online waiver. If you cannot grant this permission, contact Studio 111 LLC before booking or participating; do not sign this form. This clause does not restrict honest reviews, accident reports, communication with authorities or lawful legal claims.

9. Truthful information and electronic signature

I provide accurate identities and contact details and do not sign for someone without legal authority. The Company may suspend participation until missing or questionable authorization is resolved. Any monetary claim for a breach must be supported by the agreement and applicable law; there is no automatic unlimited forfeiture or transfer of all liability.

I have had an opportunity to read this agreement and ask questions. I intend my electronic signature to authenticate this identified version for this identified event and consent to electronic records for this transaction. I may request a copy or a paper signing option before participation. Payment by itself is not a substitute for an individual's signature.

10. Governing law and severability

Texas law governs, subject to controlling federal law. If a provision is unenforceable, it will be severed to the extent permitted by law without expanding another provision or affecting rights that cannot lawfully be waived. This draft does not impose binding arbitration or waive jury trial.
