SBA ELITE MEMBERSHIP FORM

Complete this form to join our elite boxing membership and agree to the terms. By signing this agreement, the Member confirms they have also registered with and accepted Body Elite’s online Terms and conditions. Where any terms of this agreement conflicts with on differs from Body Elite’s Terms and conditions (including but not limited to cancellation terms), Body Elite’s Term and conditions supersedes this and shall prevail.

The Elite Membership entitles the member to three (3) coached sessions per week.

- Training Days: Tuesday, Wednesday, and Thursday. Training Hours: 18:30 – 20:00.

- Curriculum: High-skills technical boxing, fitness & conditioning, and supervised sparring.

SBA operates as a specialist organization within Body Elite.

- Access Limitation: Gym access and use of equipment are strictly limited to the designated class times (from 18:30). Members are not permitted to use the facility outside of these hours under the SBA membership agreement.

- Security & Biometrics: Access is controlled via a fingerprint biometric system. It is the member's responsibility to have their fingerprint captured by the facility management during their first week. Access will only be active during the class window. For hygiene, safety, and professional standards, SBA does not provide personal protective equipment.
- Mandatory Gear: Every member is required to purchase and bring their own handwraps and boxing gloves to every session.

- Safety Gear: For sparring sessions, a mouth guard and headgear are highly recommended and may be made mandatory at the coach's discretion. - Maintenance: Members are responsible for the cleanliness and condition of their own gear. Boxing is a discipline-based sport. - Orderly Conduct: Members must follow all coaching instructions immediately. Uncontrolled aggression, ego-driven sparring, or disrespect toward fellow members or staff will not be tolerated. - Right of Removal: SBA reserves the right to remove any member from a class or terminate their membership immediately, without refund, for misconduct, bullying, or any behavior that compromises the safety and professional environment of the class. - Payment: Membership fees are payable in advance on the 1st of each month. - Notice Period: Cancellation of contract prior to the expiry date must be done by either Client or an SBA representative with written notification via their registered email address of at least 30 days in advance of the next billing date to info@bodyelite.co.za. A penalty fee calculated according to cancellation date and contract time remaining will be charged on contracts canceled before contract end date regardless of the reason for cancellation. This will be full fee for the 30 day contract cancellation notice period plus 40% of the remaining contract value plus any penalties or overdue fees outstanding. - Due Date: All membership fees are due on the 1st of each month. - Grace Period: A grace period is provided until the 3rd of the month. - Penalty: Any payment received after the 3rd, or any payment that fails due to insufficient funds (bounced), will incur a non-refundable R150 penalty fee. - Automatic freeze period: All memberships are FROZEN over DECEMBER & JANUARY as no classes will take place during the period. Classes will resume in late January. - Freeze policy: All members are allowed to freeze their memberships for a one month period, not including the automatic freeze period applied over December & January. - Contract end date: End dates move on by the same frozen period duration (e.g., your membership will end 2 months later due to automatic freeze). - Access Suspension: Failure to settle the monthly fee and the penalty by the 5th of the month will result in the immediate deactivation of the member's biometric (fingerprint) access until the account is settled in full. - CANCELLATION POLICY

Early Cancellation: If you cancel before your 6, 10, or 12-month term ends, a 40% penalty fee applies to the remaining balance (excluding the final 1-month notice period, which is payable in full).

Late-Term Cancellation: Once you pass 66% of your contract (e.g., after the 4th payment of a 6-month term), you are liable for 100% of the remaining contract value.

Payment: Fees are due immediately and will be debited automatically if unpaid.

Upfront Memberships: Cash-up-front packages are non-refundable.

By signing this agreement, the member reaffirms their acceptance of the SBA Liability & Indemnity Waiver, acknowledging the risks associated with high-skills training and sparring.
