Advanced F=ma Preparation Program Intake Form

The Advanced F=ma Preparation Program (created by Eddie Chen: www.ezchen.org) is a personalized training program for students who have a strong grasp of introductory mechanics as taught in standard physics textbooks or classes, but struggle with the F=ma exam. Such students should be able to comfortably solve mechanics questions from standard high school physics textbooks such as HRK and HRW, or have achieved a score of 5 on the AP Physics exam. Students who have already qualified for USAPhO are welcome as well, especially if they would like to increase their F=ma score. The program will not only prepare students to qualify for USAPhO, but also help students solve USAPhO mechanics problems.
Please take the time to fill out the form as accurately as possible, if you are interested in the program. It will help me understand your needs better. Thank you!

What is your math background?

Include math classes, self study, competition results, etc.

What is your physics background?

List all physics classes you took, all textbooks/resources you used and how much of the problems you can comfortably solve, all physics competition results, etc.

Which past F=ma exams have you taken (either officially or for practice)?

Check all that apply.

Of the above, which F=ma exam did you take the most recently (or that is most representative of your current ability), either officially or for practice (timed, under exam conditions)? What was your score?

Is your score on this specific F=ma exam representative of your current ability? If not, explain why.

For this specific F=ma exam, list all problem numbers that you struggled with OR did not answer correctly, for whatever reason. Select ALL that apply.

Of these problems, which of them did you struggle with because you couldn't understand the question, OR you couldn't come up with a solution within ~8 minutes, OR you came up with a conceptually incorrect solution? Select ALL that apply.

If you made a silly mistake, and could have comfortably solved the problem otherwise, don't count that.

About how many hours per week can you dedicate to this program?

Any questions or comments?

If you want to, you may also give your thought process behind some/all problems.
Thank you! Please review your responses carefully before submitting the form.
