March 29th 'Upstate Tip-Off' Tournament Sign-Up

This is the sign-up form for our first MVE hosted tournament, the 'Upstate Tip-Off'. Each team is guaranteed 2 games, with a possible third championship game (depending on record and tie-breakers). Organizations signing up multiple teams are eligible for a multi-team discount, message us on any social media or email us for more information.

Waiver & Release of Liability

By registering for the MVE 'Upstate Tip-Off' Tournament, you acknowledge and agree to the following:

Basketball is a physical sport that carries inherent risks of injury. By participating, coaches, team representatives, players, and spectators voluntarily assume all risks associated with participation, including but not limited to physical injury, accident, illness, or loss of personal property.

Mohawk Valley Elite, its organizers, directors, coaches, volunteers, staff, and any affiliated facilities shall not be held liable for any injuries, damages, losses, or claims arising from participation in or attendance at the Upstate Tip-Off Tournament, whether during games, warm-ups, or any related activities — including travel to and from the event.

Coaches and team representatives signing up on behalf of their organization confirm that:

All players are physically fit and able to participate

Their organization carries appropriate insurance coverage for their players

They have the authority to agree to these terms on behalf of their team and organization

By submitting this form, you release Mohawk Valley Elite and all associated parties from any and all claims, demands, or legal action related to participation in this event.
