Are you under the age of 16?

Has it been 6 months or more since your last colouring service or allergy test?

Have you ever had an allergic reaction to hair colourant?

Have you had an allergic reaction to a henna tattoo?

Since your last colour service, have you had an allergic reaction to any other hair products?

Do you currently have a damaged or irritated scalp?

Do you have a known allergy to any nuts?

Important Information

If, during the test or within the following 48 hours, the client experiences any signs of a reaction—such as itching, redness, swelling, burning, or irritation—the product should be removed immediately, the area rinsed thoroughly with water, and the client should seek medical advice. The colour service must not proceed.

If no reaction occurs after 48 hours, the colour treatment may proceed.

Please Note A skin tolerance test is an important safety precaution and significantly reduces the risk of an allergic reaction. However, it does not guarantee that an allergic reaction will not occur, as sensitivities can develop over time, even in clients who have previously coloured their hair without issue. Client Declaration I confirm that the information I have provided is accurate to the best of my knowledge. I understand this test is necessary to minimise allergic reactions to hair colour services, and that I must wait a minimum of 48 hours before any colour service can be carried out.
