'Super Snackers' Parent Consent & Allergy Form

Welcome to The Garden Box Super Snackers Program!

We're looking forward to providing your child with fresh, healthy produce throughout the school week.

Before your child can participate in the program and place orders through Flexischools, we kindly ask you to complete this enrolment form.

The information you provide will help us safely prepare your child's snacks and ensure we are aware of any allergies, dietary requirements or food restrictions. Rest assured that all information is kept confidential and used only for administering the Super Snackers Program.

4. Program Details

•Fruit varieties are provided on a rotating basis and are subject to seasonal availability and quality. While we aim to provide a wide variety of fresh fruit, substitutions may be made where necessary.

•Please order before 12:00 pm on Sundays. Any orders after this time will be processed for the following week.

5. Allergens

• Our fresh produce is prepared and packed with care. However, while every reasonable precaution is taken, food may be prepared, packed or handled in environments where allergens are present. As such, The Garden Box cannot guarantee that any product is completely free from allergens or cross-contamination.
• I understand that food may be prepared, stored, or handled in environments where allergens may be present, including but not limited to gluten, wheat, milk, eggs, soy, peanuts, tree nuts, sesame, and other common allergens.
• I acknowledge that despite all reasonable precautions, there remains a risk of cross-contamination.
• I understand that it is my responsibility as the parent/guardian to provide accurate and up-to-date allergy, dietary and medical information.

6. Cancellations and Refunds

• Cancellations or refund requests for weekly orders must be received at least 3 days before the start of the relevant school week.
• If you have paid for the full term and cancel before the end of the term, we will refund the remaining weeks. The weeks your child has already attended will be charged at the normal weekly rate, and the remaining balance will be refunded. • Refunds cannot be provided if a child is absent from school, as snack orders are prepared and organised in advance. However, we understand that exceptional circumstances may arise. If you believe your situation warrants special consideration, please contact us by email and we will consider your request on a case-by-case basis. • If The Garden Box is unable to supply an order due to circumstances within our control, an appropriate refund, replacement or credit will be provided.

7. Privacy and Information Sharing

The Garden Box respects your privacy and is committed to protecting your personal information.

By enrolling your child in the Super Snackers Program, you acknowledge and agree that:

•The information you provide will be used solely for administering the Super Snackers Program. •Where necessary, relevant information may be shared with Al-Taqwa College to facilitate the operation of the program. This may include your child's name, class, order information, allergy or dietary information, and any other details reasonably required to ensure the safe and efficient delivery of the program. •Your personal information will not be shared with any other third parties unless required by law or where you have provided your consent. •The form runs on Tally and responses are stored on secure servers in the EU. We share necessary details with Al‑Taqwa College to operate the program.

8. Parent/Guardian Declaration

By signing this form,

•I confirm that I am the parent or legal guardian of the child named in this form.

•I confirm that the information provided is true, complete and accurate to the best of my knowledge. •I have read, understood and agree to the information provided in this form, including the allergy information, Terms and Conditions, and the cancellation and refund policy. •I give permission for my child to participate in the Super Snackers Program and receive fresh fruit and any other food items supplied as part of the Super Snackers Program. •I understand that it is my responsibility to notify The Garden Box promptly if my child's allergy, dietary, medical or emergency contact information changes. •I understand that The Garden Box reserves the right to make reasonable changes to fruit varieties, menu offerings, delivery arrangements or program operations where necessary due to seasonal availability, supply shortages or other circumstances beyond our control. Parents will be notified of any significant changes where practicable. 
• I understand that if my child’s allergy, medical condition or dietary requirements are considered by The Garden Box to present a significant risk that cannot be safely managed within the program, The Garden Box may decline or suspend participation until a suitable arrangement can be made. • I agree that The Garden Box may contact me by email, through the Flexischools App or SMS regarding my child's participation in the Super Snackers Program, including important updates, order information, helpful reminders, additional program information and, from time to time, news, special offers and promotions. I understand that I may opt out at any time. • I understand that participation in the Super Snackers Program is subject to the policies and procedures of Al-Taqwa College where applicable. •I acknowledge that my electronic signature has the same effect as my handwritten signature for the purposes of this enrolment.
