I understand that participating in a walk/run event involves inherent risks, including but not limited to falls, contact with other participants, traffic, weather conditions, uneven surfaces, and other hazards that may result in serious injury, illness, property damage, or death.
I certify that I am physically fit and have sufficiently trained to participate in this event. I have not been advised by a qualified medical professional not to participate, and I assume full responsibility for my participation.
In consideration of being permitted to participate in the Project Heartburn 5K Walk/Run, I, on behalf of myself, my heirs, executors, administrators, and assigns, voluntarily release, waive, discharge, and hold harmless United Way of Lea County, the City of Hobbs, event sponsors, volunteers, organizers, officials, employees, agents, contractors, and all other persons or entities associated with this event from any and all claims, liabilities, demands, damages, causes of action, costs, or expenses arising out of or related to my participation, including those resulting from ordinary negligence.
I understand that emergency medical services may be provided if deemed necessary. I authorize emergency medical treatment if I am unable to provide consent, and I accept responsibility for any medical expenses incurred.
I grant permission to United Way of Lea County and the City of Hobbs to use photographs, video recordings, or other images of me taken during the event for promotional, educational, marketing, and fundraising purposes without compensation or further permission.
I understand that my registration is nonrefundable and nontransferable unless otherwise determined by the event organizers. The event may be delayed, modified, or canceled due to severe weather, emergencies, or circumstances beyond the organizers' control.
If I am registering a participant under the age of 18, I certify that I am the parent or legal guardian of that participant and agree to this waiver on their behalf.
