I authorize Furever Fluffy LLC to groom my dog. I have shared relevant health, behavior, and handling concerns and will tell the groomer if anything changes before a future appointment. Grooming may involve bathing, drying, brushing, nail care, and clipping or trimming as appropriate.
If my dog has painful matting, skin irritation, fleas, or becomes too distressed or unsafe to handle, the groomer may pause or stop the service. The groomer will contact me to discuss a shorter cut, additional time or charges, or an incomplete groom. Mat removal or a shorter cut can reveal existing skin irritation.
Please call me first when reasonably possible. If urgent care cannot wait or I cannot be reached, I authorize Furever Fluffy LLC to transport my dog to Animal Emergency Hospital DeLand, 2100 E New York Ave, DeLand, FL 32724, (386) 252-0337, and seek veterinary evaluation. If a grooming-related injury requires an emergency visit, Furever Fluffy LLC will cover that emergency visit, as stated in my prior agreement.
Please give at least 24 hours notice to cancel or reschedule; a late cancellation fee may apply. Pick up your dog within 2 hours after we tell you the groom is complete; an additional boarding fee may apply after that. If behavior prevents us from completing the groom, you may owe a charge for work already performed. Any fee amount will be disclosed before it is charged.
