RELEASE AND WAIVER OF LIABILITY, INDEMNITY AND ASSUMPTION OF RISK AND PARENTAL CONSENT AGREEMENT IN CONSIDERATION of being permitted to participate in any way in SHERIDAN TACTICAL and any of its activities or events, including but not limited to paintball, tactical training, or other activities (referred to as “activities”) and including, but not limited to, playing, training, renting and using leased equipment, and use of the premises. I for myself, and/or the participant(s) I am authorized to sign on behalf of,
ACKNOWLEDGE and agree that:
I understand the nature of Sheridan Tactical and its activities and events, and that it requires high physical activity and I am qualified, in good health, and in proper physical condition to partake in these activities.
I understand that participation in any event or activity at Sheridan Tactical (with or without the use of rental equipment) may result in injury and/or illness, including but not limited to, bodily injury, disease, fractures and strains, paralysis, eye injury or blindness, heart attack, heat stroke, seizures, death (wrongful or otherwise), or other ailments that can cause disabilities, permanent or otherwise. And, therefore, I assume the inherent and other risks that may result in harm, injury, illness, loss, or death during my participation in any activities or events. I fully accept and assume the risks involved and take accountability for any damages, losses, and/or expenses that might be incurred during and as a result of my involvement in the activities or any event.
I agree to listen to and abide by the rules and safety policies put in place by Sheridan Tactical and its employees, as well as obey any and all signage posted in the facility. I understand I may be recorded by cameras on the facility for safety purposes, as part of the everyday routine on the facility.
I understand that I have the right to withdraw anytime during the event or any activity in which I am a participant of, should I believe or think the conditions in the event endangers my safety in any way. I agree to immediately discontinue further participation in any activities if I do think my safety is at risk.
I understand the risks involved, known and unknown, foreseeable or not foreseeable, in participation in any activity or event, which includes but is not limited to injury, disability or death whether caused by accident, by negligence of the owners, employees, participants, or anyone else, or by forces of nature, weather, acts of god, alleged or actual transmission of communicable disease, and/or any other causes.
Furthermore, I give MEDICAL TREATMENT CONSENT, hereby giving permission for Sheridan Tactical to authorize any emergency medical treatment that is believed to be necessary during my participation in any activities or events and/or with any equipment, rented or otherwise.
I hereby voluntarily agree to waive, release, discharge, covenant not to sue, indemnify and hold harmless Sheridan Tactical, its owners, landowners, directors, administrators, agents, volunteers, operators, its officers and employees, the sponsors, advertisers, and the venue of the event or activities in any incident resulting to loss, liability, damage or damages, death, claims, causes of action whether in whole or in part, the negligence by which may be incurred, including negligent rescue operations. I agree not to hold Sheridan Tactical under any liability whatsoever at any point in time.
If I, or anyone on my behalf, makes a claim against Sheridan Tactical or any of its owners, employees or other personnel, despite this waiver, I will indemnify, save, and hold harmless said members from any litigation expenses, attorney fees, losses, liability, damage or anything else that incurs as result of such claim.
I hereby sign this form freely, unconditionally, and without inducement or consideration of any nature as an affirmation and consent to the contents of this waver and release by which I will be legally bound. I fully understand the terms of this waiver and understand I have given up substantial rights by signing it.
Participation in any activities in Sheridan Tactical is your unconditional release of liability regardless of whether or not this waiver is signed. By not signing this waiver, or by signing it under a false name, I agree that I have participated in activities or events at Sheridan Tactical without permission and without safety protocols, resulting in myself being at fault and completely assuming all responsibility for any and all damages and liability, as written and outlined in this waiver.
Sheridan Tactical reserves the right to refuse service to anyone at any time.
I give my consent to allow Sheridan Tactical to use videos and/or photos of me taken at the facility and related events to promote, advertise, and use on social media.
MINOR RELEASE AND PARENTAL CONSENT: I, the minor’s parent and/or legal guardian, understand the nature of Sheridan Tactical activities and equipment, and the minor’s experience and capabilities, and hereby agree to the agreement above in its entirety.
