Welcome to Tended.

Finding a caregiver who fits your family is about more than availability. It begins with understanding your children, your family's rhythm, the support you need, and the kind of relationship you hope to build.

This questionnaire is the first step. It helps Tended get to know your family, understand the role you're hoping to build, and determine whether we're the right fit to support your search.

Please answer as openly and specifically as you can. Thoughtful, honest information at the beginning helps us create stronger roles, make more intentional introductions, and set everyone up for a better working relationship.

Your responses are handled in accordance with our Privacy Policy. This questionnaire takes approximately 10 minutes to complete.

What happens next?

1. Complete this questionnaire.

2. Tended will review your family and care needs within 3 business days.

3. If we believe we're well aligned, we'll invite you to take the next step with Tended.

We know inviting someone into your home is deeply personal. Our goal is to make the process feel thoughtful, clear, and well supported from the start.

Understanding Your Family's Care Needs

Every child and family is different. The more we understand the care environment from the start, the better we can find caregivers who feel genuinely prepared and aligned.

Let's Start with the Basics

What is your full name?

What is your email address?

What is your phone number?

Tell us about your child (ren). Please include their name(s), age(s), and pronouns

What neighborhood do you call home?

Please share anything you'd like us to understand about your child(ren)'s individual needs, temperament, or the kind of support that helps them thrive. This might include medical or developmental needs, neurodivergence, sensory or emotional needs, allergies, behavioral or safety considerations, or anything else that would help us thoughtfully shape the role and find a caregiver who feels genuinely prepared and aligned. The more context we have, the better we can support both your family and the caregiver from the beginning

What Matters Most to You

Every family is different. Help us understand what success looks like for yours.

What are the three most important qualities you're looking for in a caregiver?

How would you describe your parenting style and the kind of relationship you hope your caregiver will have with your family?

Are there any specific experiences, skills, or qualifications that are especially important for this role? For example: infant or newborn experience, experience supporting neurodivergent children, strong swimmer, bilingual, travel experience, confident driver, etc.

Almost Done!

How Tended Works

Tended begins with clarity, not the search. We take the time to understand your family, thoughtfully shape the role, and then search for caregivers who feel genuinely aligned.

Consultation & Engagement — $500. If Tended and your family decide to work together, you’ll begin by reviewing and signing Tended’s Family Placement Agreement and paying the nonrefundable $500 Consultation & Engagement Fee. This fee is separate from and is not credited toward the Placement Fee. From there, we’ll schedule your Family Consultation, thoughtfully develop your role, and prepare your personalized search.

Placement Fee — 10% of your caregiver's projected first-year gross compensation, with a $5,000 minimum. Includes hiring and onboarding guidance and 60 days of post-placement support.

Once your role is ready to launch, your initial active search runs for six weeks. Tended makes fewer, more intentional introductions rather than promising a set number of candidates.

The Family Placement Agreement includes full terms, including payment, search extensions, and placement support, before your engagement begins.
