Access request ticket

Rendering Consulting Inc. requires prior request and approval before granting access to internal systems. Before onboarding is complete, HR/IT/managers should submit on behalf of new hires and contractors.
Form structure: Part 1 is request type and dates, Part 2 is submitter, Part 3 is person who needs access, and Part 4 is system details.

Policy

- Do not grant system access before HR onboarding is complete (e.g., employment contract, IP/NDA, information security policy acknowledgement as applicable).
- Principle of least privilege — limit access strictly to what is required for the role.
- Do not grant access before the official employment/contract start date.
- Do not grant access without approval from each system/data owner or responsible manager.
