-I understand that this procedure is semi permanent, and the gem is bonded to enamel with dental adhesives.
- I agree that I do not require any dental work on the tooth where the gem will be placed.
- I understand that if I have any plaque or tartar buildup on my teeth, the tooth gems will not adhere properly and no warranty will be provided. A cleaning is always recommended before tooth gems.
- I understand that cherry nails uses similar techniques and similar bonding materials that orthodontists used to apply braces.
- I understand Sophie will do her best to ensure your tooth gems have maximum retention.
- I understand all instruments are either single use, individually packaged, or sterile.
- I understand that if I wear a mouthguard, Invisalign, or a retainer where the gems will be placed, tooth gems may not be suitable for me. Sophie will do their best to place where it will not interfere but if you do have one of these, please bring them to your appointment.
- I understand that if I whiten my teeth or stain my teeth while wearing the tooth gem that the area beneath my gem may be a different colour.
- I understand that if I whiten my teeth 2 to 3 weeks prior to getting a gem, my retention will not last long.
- I understand that if the gems fall off after ONE MONTH warranty that I am solely responsible for the cost of replacing them or removing any adhesive that may be remaining at the dentist.
- I understand tooth gems are made of small lead free crystals attached to a foil backing using a special adhesive. Sometimes the tooth gem may separate from the foil backing due to a variety of reasons. If this happens, I understand it will not cause any harm and should be removed by your dentist.
- I agree to let Sophie know if I have a filling on the tooth where I want a gem.
- I agree that I am 16 or older.
- I consent to the application of the gems and to any actions that are reasonably necessary to perform the procedure.
- I consent to pictures being taken and posted to Cherry nails Instagram or website.
- I acknowledge that by signing this agreement that I have been given the full opportunity to ask any and all questions and that all of my questions have been answered to my full satisfaction.
