Thanks for your interest in a Comprehensive Diagnostic Assessment (CDA). This short form helps us both make sure this service is the right fit before we schedule.
The assessment. A CDA is a snapshot of your current mental health, based on an intake interview and my clinical judgment. It typically takes about 1 hour. Appointments are available Tuesday through Friday, 10am to 4pm CT.
Fee and booking. The CDA is $350, paid in advance. If we're a good fit, I'll send you a link to pay and book your appointment yourself.
Your report. You'll receive your completed CDA as a PDF within 3 business days, and usually within 1.
What's included, and what isn't. I guarantee the assessment and the written report only. I can't promise any particular diagnosis or outcome, including FMLA approval, FAA clearance, benefits, or accommodations. Those decisions are made by your employer, the FAA, an insurer, or another third party. Diagnosis is based on my clinical judgment rather than self-assessments, and I don't provide formal ADHD evaluations, which require a psychological testing center. I also don't provide court testimony or appear in legal proceedings.
Location. I'm licensed in Idaho, California, and Texas, so I can only provide assessments to people located in one of those states at the time of the appointment.
