Melbourne’s Many Faces: Walking Discovery Tours Registration Form

I have read the Risk Waiver and Release of Liability form and understand that the parent / guardian of each student and all adults will be required to sign  and provide the form to Schools in Action to be eligible to participate on the Tour.

I have read and understand the Tour Code of Conduct. I will ensure all participants will read the document prior to the tour and abide by the Code.

I acknowledge and accept that the school will be invoiced a deposit of $100 (GST exclusive) for registering for the walking tour as per the Tour Terms.
