CLIENT CONSENT, RELEASE OF LIABILITY, AND MEDIA RELEASE
I, the undersigned, hereby consent to receive mobile barbering services from NeCutz Mobile Barbering. I acknowledge and agree to the following:
1. Assumption of Risk: I understand that haircutting involves the use of sharp tools (clippers, scissors, razors) and that minor nicks, cuts, or skin irritation are a normal risk of the service. I voluntarily assume all such risks.
2. Health Disclosure: I confirm that I have no contagious skin conditions (e.g., ringworm, impetigo, lice) or open wounds, and I have disclosed any known allergies or skin sensitivities to the barber prior to the service.
3. Home Environment: I confirm that the location I have provided for this service is safe, well-lit, and free of hazards. I understand that NeCutz is not responsible for damage to my property caused by normal haircutting activities (e.g., loose hair clippings, water spills, minor product drips). I agree to hold NeCutz harmless from any claims related to my home environment.
4. Release of Liability: I hereby release NeCutz Mobile Barbering, its owner, and its employees from any and all liability for injuries, damages, or losses arising from the service, except in cases of gross negligence or willful misconduct. I agree to indemnify and hold NeCutz harmless against any claims resulting from my failure to disclose a health condition, allergy, or unsafe home environment.
5. Deposit & Cancellation: I understand that the $15 deposit is non-refundable and applied toward my total. Cancellations within 24 hours of the appointment forfeit the deposit.
6. Media Release (Optional): I grant NeCutz Mobile Barbering permission to take before-and-after photographs of my haircut for social media and marketing purposes. I understand I may opt out by checking the box below.
