I am the parent or legal guardian of the minor named above and give permission for the minor to participate in the Activity.
I understand that the Activity involves risks, including falls, collisions, equipment malfunction, weather conditions, travel, actions of other participants, and other known or unknown risks that could result in injury, illness, property damage, disability, or death. I have explained, or will explain, appropriate safety rules to the minor.
If I cannot be reached in an emergency, I authorize the Activity organizers to obtain reasonable emergency medical evaluation, treatment, and transportation for the minor. I understand that I am responsible for related costs to the extent not covered by insurance or otherwise required by law.
To the fullest extent permitted by law, I release and agree not to hold responsible Oaklyn Civic Association, its organizers, officers, employees, volunteers, hosts, sponsors, and venue owners (collectively, the “Released Parties”) for claims or losses arising from the minor’s participation in the Activity, including claims based on ordinary negligence. This release does not apply to conduct that cannot legally be waived, including gross negligence or intentional misconduct.
All participants, and parents/guardians of minor participants, agree to the following:
Follow instructions. Participants must follow organizer, staff, volunteer, venue, and safety instructions at all times.
Act safely and respectfully. No harassment, discrimination, bullying, threats, fighting, reckless conduct, or behavior that puts others at risk.
Use required equipment. Participants must use any required safety equipment correctly and may not use damaged or unsafe equipment.
Report hazards and injuries. Participants must promptly report unsafe conditions, injuries, illness, damaged equipment, or rule violations to an organizer.
Health and fitness. Participants are responsible for deciding whether they are physically and medically able to participate. Participants should stop and notify an organizer if they feel unsafe, injured, ill, dizzy, or unable to continue.
Weather and schedule changes. Organizers may delay, modify, suspend, or cancel the Activity for weather, safety, attendance, venue, or operational reasons.
Personal property. Participants are responsible for their own belongings. The organization is not responsible for lost, stolen, or damaged property except where required by law.
Removal from activity. Organizers may remove a participant from the Activity for unsafe, disruptive, or noncompliant behavior. Fees, if any, may be nonrefundable unless the organization states otherwise.
Photos and recordings. I allow Oaklyn Civic Association to use photographs, video, or audio of my minor participant for reasonable organization communications and promotion, without payment.
