[GH2] Makestar 6 Raffle

Hello everyone! Thank you so much for joining my group order for Makestar

This group order will collect initial payment first, and ems / dom later.

Please make sure to check the masterlist. If there are any mistakes with the claims, let me know immediately before filling up the form.

Payment is due by March 4th 6PM PST.

I need to purchase it immediately, therefore I will be collecting applicant's info beforehand.

There will be NO extensions for this group order as this is a fancall raffle.

NO FORM = NO ENTRY.

If there is no communication between the joiner and the GOM, I will automatically remove you from the GO. Reminders will be sent in the group chat for payment.

FANCALL RAFFLE:

1. Please note that some stores may require passport. However, it really depends on the staff. State ID should still be okay, however, I would prefer it if you have a passport so there are no issues later on!

2. Please be available during the day of the call. The time is usually within 10PM PST to 4AM PST. If you are unavailable during these times, please do not join the raffle.

3. Raffle includes only the call and signed album. I will be sending your signed album when it gets to me. EMS and Domestic fees will be collected.

4. If possible, please post proof of fancall or any proof that you can tag me for my giveaway proofs!

5. I draw using the wheel spinner. The time I spin the wheel is usually random since my work and sleep schedule tend to be all over the place. I do try to pick the raffle winner through Instagram live, however, this is not always the case. If I am unable to start the live, I will be screen-recording the wheel spinning and send this video in the chat or my story.

By filling out this form, you confirm your agreement to the following terms and conditions:

No Refunds: Participation in this group order signifies that the specific claim is reserved for you. By claiming an item, you acknowledge and accept the stated price and all conditions associated with the group order.

No Backouts: Once you join, withdrawal is not permitted. If you require a replacement, you must coordinate with me in advance.

Timely Payments: You agree to make payments on time. If you require an extension, it is your responsibility to communicate with me promptly.

Item Holding Policy: Once the items are in hand, they will be held for a maximum of one month. A shipping request form will be provided, and you are required to complete it within 30 days. Failure to do so will result in the forfeiture of your claim, and the item will not be shipped or refunded. Group chat reminders and individual messages will be sent to ensure everyone is informed.
