Thank you for participating in APWU Local 7033's "Eight Weeks of Safety." Please use this form to submit a photo of your PS Form 1767 if you do not have point person available and you are submitting it to your supervisor on your own. Check with your steward (listed
here) to find your point person.
1. Check
here to find the informational sheet provided for the safety focus of the week.
2. Write a PS Form 1767 about one or more violations pertaining to the safety focus from the informational sheet or add your own.
3. If a point person is unavailable, take a photo of your PS Form 1767. Be sure the form is laying flat and is well lit.
4. Give your PS Form 1767 to your supervisor.
5. If your supervisor gives you a signed copy, take a picture of your signed copy and upload it right away. If your supervisor does NOT give you a signed copy in 24 hours, wait for the 24 hours to pass and upload it to this site indicating that they did NOT give you a signed copy.
