The undersigned is the parent or legal guardian of the student(s) named in this form (individually and collectively, the “Participant”). The undersigned hereby gives permission for the Participant to attend and participate in the trip/event named above (collectively and including without limitation related travel, the “Trip”), sponsored by the Church (as defined below). In consideration of the Church allowing the Participant to participate in the Trip and other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the undersigned hereby agrees as follows:
아래 서명인은 본 양식에 기재된 학생(들)(개별적으로 및 통칭하여, 이하 “참가자”)의 부모 또는 법정 보호자입니다. 서명인은 참가자가 교회(아래 정의)가 주관하는 위 여행/행사(관련 이동을 포함하되 이에 국한되지 않으며, 이하 통칭 “여행”)에 참석하고 참여하는 것을 허락합니다. 교회가 참가자의 여행 참여를 허락하는 것과 기타 정당하고 가치 있는 대가를 받았음을 확인하며, 서명인은 다음 사항에 동의합니다.
ACKNOWLEDGMENT AND ASSUMPTION OF RISK · 위험의 인지 및 수용
The undersigned understands that participation in the Trip involves inherent risks, including but not limited to travel-related risks, environmental conditions, interactions with members of the public, physical activity, and risks associated with the location and activities involved.
The undersigned acknowledges that these risks may result in injury, illness, property damage, or other harm, including serious injury or death, and that such risks may arise from foreseeable or unforeseeable causes.
With full knowledge of these risks, I voluntarily allow the Participant to participate and assume responsibility for all risks associated with participation, whether known or unknown, inherent or incidental.
서명인은 여행 참여에 이동 관련 위험, 환경 조건, 일반인과의 접촉, 신체 활동, 장소 및 활동과 관련된 위험 등을 포함하되 이에 국한되지 않는 고유한 위험이 따른다는 것을 이해합니다.
서명인은 이러한 위험이 중상 또는 사망을 포함한 부상, 질병, 재산 피해 또는 기타 손해를 초래할 수 있으며, 예견 가능하거나 예견할 수 없는 원인으로 발생할 수 있음을 인정합니다.

LIABILITY RELEASE · 책임 면제
1. To the fullest extent permitted by applicable law, the undersigned, on behalf of myself, the Participant, and our respective heirs, executors, and assigns, does hereby irrevocably release and discharge Assemblies of God Promise Church, Full Gospel New York Church-Assemblies of God, and Promise Ministries International, and their respective successors and assigns (collectively, the “Church”), together with the Church’s and the Trip’s pastors, lay leaders, officers, employees, and volunteers (collectively and together with the Church, the “Releasees”), from any and all liability, lawsuits, claims, causes of action, or demands for damages (collectively, “Claims”) arising from or related to the Participant’s participation in the Trip, including but not limited to personal injury, bodily injury, sickness, death, loss of property, property damage, or any other costs or expenses, except to the extent such Claims are found to arise from the gross negligence or willful misconduct of the Releasees.
1. 관련 법률이 허용하는 최대 범위 내에서, 서명인은 본인, 참가자 및 각자의 상속인, 유언집행인, 양수인을 대리하여 Assemblies of God Promise Church, Full Gospel New York Church-Assemblies of God, Promise Ministries International 및 각 승계인과 양수인(이하 통칭 “교회”), 그리고 교회 및 여행의 목회자, 평신도 지도자, 임원, 직원, 자원봉사자(교회와 함께 이하 통칭 “면책 대상자”)를, 참가자의 여행 참여로 인해 또는 이와 관련하여 발생하는 신체 부상, 질병, 사망, 재산 손실 또는 손상, 기타 비용을 포함하되 이에 국한되지 않는 모든 책임, 소송, 청구, 소인 또는 손해배상 요구(이하 통칭 “청구”)로부터 취소 불가능하게 면제합니다. 단, 해당 청구가 면책 대상자의 중과실 또는 고의적 위법행위로 인해 발생한 것으로 판명되는 경우는 예외로 합니다.
2. In the event of an accident, injury, or illness, I authorize Church or Trip personnel to obtain medical care for the Participant, including first aid, emergency treatment, dental treatment, hospitalization, anesthesia, and surgery, if deemed necessary by a licensed medical professional. I understand that reasonable efforts will be made to contact me in the event of a medical emergency, but that treatment may proceed if I cannot be reached promptly. The undersigned further releases the Releasees from any Claims whatsoever arising from any such medical care rendered to or on behalf of the Participant during any portion of the Trip. The undersigned agrees to bear the cost of such medical care and to indemnify and hold the Releasees harmless from any related Claims.
2. 사고, 부상 또는 질병이 발생한 경우, 본인은 면허를 가진 의료 전문가가 필요하다고 판단할 때 교회 또는 여행 담당자가 참가자를 위해 응급처치, 응급 치료, 치과 치료, 입원, 마취 및 수술을 포함한 의료 조치를 받도록 하는 것을 허락합니다. 응급 상황 시 본인에게 연락하기 위해 합리적인 노력을 기울이겠지만, 본인과 즉시 연락이 닿지 않을 경우에도 치료가 진행될 수 있음을 이해합니다. 서명인은 여행 기간 중 참가자에게 또는 참가자를 위해 제공된 의료 조치로 인해 발생하는 모든 청구로부터 면책 대상자를 추가로 면제합니다. 서명인은 해당 의료비를 부담하며, 관련 청구로부터 면책 대상자를 보상하고 면책할 것에 동의합니다.
3. The undersigned acknowledges and agrees that neither the Church nor the Trip will provide medical or health insurance coverage for the Participant, and that the undersigned is solely responsible for arranging and paying for any such insurance coverage.
3. 서명인은 교회나 여행 측이 참가자에게 의료 또는 건강 보험을 제공하지 않으며, 그러한 보험의 가입 및 비용은 전적으로 서명인의 책임임을 인정하고 동의합니다.
4. The undersigned agrees, on behalf of myself and the Participant, to be responsible for any damage to, or loss of, property belonging to the conference venue, hotel, or other facilities used in connection with the Trip that is caused by the Participant, including any related costs for which the Church or the Releasees may otherwise be held responsible.
4. 서명인은 본인과 참가자를 대리하여, 참가자로 인해 발생한 수련회 장소, 호텔 또는 여행과 관련하여 사용되는 기타 시설의 재산 손상 또는 손실에 대해, 교회 또는 면책 대상자가 부담하게 될 수 있는 관련 비용을 포함하여 책임질 것에 동의합니다.
5. To the extent any release set forth above is found to be unenforceable as to Claims brought by or on behalf of the Participant (including after the Participant reaches the age of majority), the undersigned agrees to indemnify, defend, and hold harmless the Releasees from and against any such Claims, to the fullest extent permitted by applicable law.
5. 위의 면책 조항이 참가자에 의해 또는 참가자를 대리하여 제기된 청구(참가자가 성년이 된 이후를 포함)에 대해 집행 불가능한 것으로 판명될 경우, 서명인은 관련 법률이 허용하는 최대 범위 내에서 그러한 청구로부터 면책 대상자를 보상, 방어하고 면책할 것에 동의합니다.
EARLY RETURN HOME POLICY · 조기 귀가 규정
I understand that the Participant is expected to comply with all Trip rules, instructions of supervising personnel, and applicable laws during the Trip. Should it be necessary for the Participant to return home for medical reasons, disciplinary action, or otherwise, the undersigned agrees to assume all responsibility for all transportation arrangements and cost.
본인은 참가자가 여행 기간 동안 모든 여행 규칙, 담당자의 지시 및 관련 법률을 준수해야 함을 이해합니다. 의료적 사유, 징계 조치 또는 기타 사유로 참가자가 귀가해야 할 경우, 서명인은 모든 교통편 마련과 비용에 대한 책임을 부담하는 데 동의합니다.
TRANSPORTATION · 교통
The undersigned hereby gives permission for the Participant to ride in any vehicle driven for purposes of the Trip while attending and participating in the Trip. The Participant and the undersigned understand and agree that seat belts must be worn at all times during transportation for the Trip. The undersigned further acknowledges that travel times, routes, and schedules may change due to weather, traffic, or other circumstances, and the undersigned consents to reasonable modifications made by Church or Trip personnel in the interests of student safety.
서명인은 참가자가 여행 기간 중 여행 목적으로 운행되는 모든 차량에 탑승하는 것을 허락합니다. 참가자와 서명인은 여행 중 이동 시 항상 안전벨트를 착용해야 함을 이해하고 동의합니다. 또한 서명인은 날씨, 교통 또는 기타 상황으로 인해 이동 시간, 경로 및 일정이 변경될 수 있음을 인정하며, 학생의 안전을 위해 교회 또는 여행 담당자가 하는 합리적인 변경에 동의합니다.
PHOTOS AND VIDEOS · 사진 및 영상
The undersigned consents, on behalf of myself and the Participant, to allow the Releasees to record during the Trip, and to display and use video, still images, or audio of the student(s) (“Recordings”), without payment of any compensation or royalties. I understand that any such Recordings shall be used only for legitimate religious or educational purposes related to the Church or the Trip.
서명인은 본인과 참가자를 대리하여, 면책 대상자가 여행 중 학생(들)을 촬영·녹음하고 그 영상, 사진 또는 음성(이하 “녹화물”)을 보상이나 로열티 지급 없이 게시 및 사용하는 것에 동의합니다. 본인은 이러한 녹화물이 교회 또는 여행과 관련된 정당한 종교적 또는 교육적 목적으로만 사용된다는 것을 이해합니다.
SEVERABILITY AND GOVERNING LAW · 분리 가능성 및 준거법
If any portion of this agreement is held by a court of competent jurisdiction to be invalid or unenforceable, the remaining provisions shall remain in full force and effect. This consent and release form shall be governed by and construed in accordance with the laws of the State of New Jersey, except to the extent the same would direct the application of the laws of another jurisdiction.
본 동의서의 일부가 관할 법원에 의해 무효 또는 집행 불가능한 것으로 판단되더라도, 나머지 조항은 계속 완전한 효력을 유지합니다. 본 동의 및 면책동의서는 뉴저지주 법률에 따라 규율되고 해석되며, 다른 관할권의 법률 적용을 지시하는 경우는 예외로 합니다.
ENTIRE AGREEMENT · 완전 합의
This document constitutes the entire agreement regarding the Participant’s participation in the Trip and supersedes any prior oral or written understandings related thereto.
