What is your first name?

What is your email address

What is today's date

Who do you share your home with?

1. What has prompted you to seek a Home Therapy Diagnostic Consultation now? (Timing matters more than history.)

2. In one or two sentences, how would you describe the current relationship between you and your home? (For example: supportive, draining, confusing, disconnected, unresolved.)

3. Which area of your home currently feels most problematic — and why? (One room or zone only.)

4. Is there a space in your home that makes you feel instantly calmer when you walk into it? Or more anxious?

5. How does this show up in your body, mood, or behaviour?

(Sleep, energy, focus, irritability, avoidance, overwhelm, etc.)

6. What feels most “out of alignment” for you at the moment?

(This may be practical, emotional, or habitual.)

7. Thinking about your home, do you have at least one space where you get natural light in the morning?

8. Are there rooms where artificial light feels harsh or particularly draining?

9. When you are at home, do you regularly open windows or doors to let in fresh air? Can you identify any sources of stale air, mustiness, mould or damp?

10. Do you feel more energised or sluggish after a day spent at home? Clear-headed or foggy after being indoors for long periods?

11. Can you walk freely around your space without moving things around?

12. To the best of your awareness, are you holding onto anything out of guilt, habit or avoidance? Are there any items you avoid looking at because they trigger unwanted emotions?

13. Do you have many objects around you that make you feel proud, peaceful, or inspired?

14. Do you have a quiet corner for calm that you can call your own, however small? Does your home allow space for silence, or is it always "on"?

15. Are there any consistent sounds that irritate you at home, chargers, buzzing lights, ticking clocks, noise bleed?

16. What have you already tried in an attempt to improve things?

(Briefly — this helps identify patterns, not effort.)

17. Are there any current life transitions or pressures that feel relevant?

(Health, work, relationships, grief, change, recovery, caregiving, etc.)

18. What would better actually feel like for you in daily life?

(Not what your home would look like — how you would feel.)

19. Is there a habit that you want to build (eg reading, exercise, journalling) that your home currently makes harder rather than easier?

20. Does your home reflect who you are now, or someone you used to be?

21. What are you hoping this consultation will help you to understand or see more clearly? What would make this consultation feel genuinely worthwhile for you?(Clarity is the goal, not solutions.)

This consultation is diagnostic rather than design-led. Are you comfortable with guidance that focuses on insight, prioritisation and direction rather than specific products or finishes?
