Thank you for helping us build your athlete's recruiting profile. This form collects the base information, photos, videos, and permissions needed to prepare the website. A parent or legal guardian should complete this form. Invoice and payment will be handled after the request is reviewed.

Shared Albums Guide

Invidual Photos/Videos
Open the Photos app.Select the photos/videos you want to share. Tap Share. Choose “Copy iCloud Link” or save them to a shared iCloud Drive/Google Drive folder. Paste the link in this form. Make sure the link can be opened by someone else.


Android

On your mobile device, open the Google Photos app. Sign in to your Google Account. Select a photo, album, or video. Tap Share. To create and share a link, tap Create link.


Important

I recommend sending extra photos, and at most 12 short videos. Each video should be 5-30 seconds long. Videos can be in any format; we will compress them for the website. Please avoid sending screenshots of videos. Please only send photos/videos you approve for the website.
