Lakewood 6 & 7 — Architectural Control Committee (ACC) Request

Use this form to request ACC review for any exterior change visible from the street or common area (CC&Rs 6.3). Please allow up to 30 days for review. Note: the HOA does not obtain or approve City of Boise permits — meeting City requirements remains the homeowner's responsibility.

Please include the following in your description, depending on your project:

Fence — construction material, height, exact location, and attachment point (front or back of home).

Landscaping — design (attach plans below) and a plant list (trees with caliper; plants with container size, e.g. 1/2/3/5 gallon).

Paint — manufacturer and color name/number so it can be viewed on the manufacturer's website. A physical sample may be requested at the ACC's discretion.

Roofing — must be a board-approved material: CertainTeed Presidential, CertainTeed Landmark TL, GAF Grand Sequoia, Malarkey Windsor, or Owens-Corning Duration Max.
