Youth players participating in the New York Rugby Sevens Tournament must have a parent or legal guardian complete the required waiver and consent forms.
These documents cover tournament participation, medical treatment permissions, and optional injury research conducted by Hospital for Special Surgery in partnership with NY7s.
Forms must be completed before a minor is eligible to compete.
A parent/guardian of each player must sign these forms.
By doing so each parent/guardian, on behalf of the player for which they sign, affirms, agrees, acknowledges and ratifies that they have read the 2025 NY7s TOURNAMENT PARTICIPATION, WAIVER & RELEASE FORM following this roster list, and that they are a signatory to such Participation, Waiver & Release form and are bound thereby. Each parent/guardian also affirms, agrees, acknowledges, ratifies, represents and warrants that have the authority to sign and enter into this Participation, Waiver & Release form for and on behalf of the player for which they sign.
