Carpool Request Form

Hello! You are invited to participate in a volunteer-led carpool for the Bearing Witness standouts on Wednesdays in Burlington MA. We are currently organizing rides from a few nearby parking lots, with limited availability. Signing up in advance is encouraged. If you would like to sign up, and share your contact information below, we will reach out with additional information. Your participation is voluntary, and you may choose not to answer any questions or stop the survey at any time without penalty. We will only use your response for planning and scheduling purposes.
