Tended Caregiver Questionnaire

Welcome to Tended. We're so glad you're here.

At Tended, we believe exceptional caregiving relationships begin with thoughtful introductions, clear expectations, and genuine mutual respect.

We know how much heart, skill, and professionalism exceptional caregivers bring to their work. That's why we take the time to personally get to know every caregiver and thoughtfully curate the Tended Collective.

This questionnaire helps us understand your experience, what you're looking for, and the kind of family where you'll feel most supported and thrive.

We keep everything you share confidential and use it only to support the Tended matching process.

Thank you for sharing your story. We're looking forward to getting to know you.

Estimated time: 8–10 minutes.

Before You Begin

To join the Tended Collective, caregivers will need:

✓ TrustLine registration (or be actively applying)

✓ Infant & Child CPR certification before placement

✓ At least one professional childcare reference

Don't have everything yet? That's okay.

If we think you're a strong fit, we'll help guide you through the remaining steps before introducing you to families.

TrustLine Status

TrustLine registration is required before a caregiver can be introduced through Tended.

If you're already registered, wonderful; please upload your documentation below.

If you're not yet registered, that's okay.

Learn more about why it's required and how to get started in our Understanding TrustLine: A Guide for Tended Caregivers.

TrustLine is California's background check program for in-home childcare providers. As a licensed California referral agency, Tended must verify each caregiver's TrustLine status before introducing them to families. This isn't unique to Tended—it's a legal requirement for licensed referral agencies throughout California.

Current CPR Certification

If you already have a current Infant & Child CPR certification, please upload it below.

Infant & Child CPR certification is required before Tended can place a caregiver.

We recommend certification through either the American Heart Association or the American Red Cross, though we also accept equivalent nationally recognized certifications.

Please upload your current resume if you have one

A simple, up-to-date resume is perfect. Please include your childcare experience, approximate employment dates, the ages of children you've cared for, and your primary responsibilities in each role.

Professional References

Professional references help us understand your experience and are an important part of Tended's screening process.

Please include the following for at least two professional childcare references:

• Reference name

• Relationship to you (parent, employer, supervisor, etc.)

• Family or employer name

• Ages of the children you cared for

• Approximately when you worked together

• Phone number and/or email address

• Preferred contact method

• Whether Tended may contact them now

Tended keeps reference information confidential and uses it only for our screening and placement process.

Optional: Upload Written References/Recommendation Letters.

If you have written reference letters or recommendation letters from previous families, you're welcome to upload them here. This is completely optional but can help us get to know you and your experience.

Is there anything else you'd like us to know about yourself or the type of family you're hoping to work with?

Please upload a photo of yourself.

This helps us create your Tended caregiver profile and allows families to begin each introduction with a sense of familiarity and connection. Your photo will only be shared with families after we've spoken with you and believe there may be a strong mutual fit.

How were you referred to Tended?

Tended grows primarily through trusted referrals. We'd love to thank the person who connected us.
