Client questionnaire

Therapy is often a lot of wasted time on both sides. I like efficiency and being straightforward with you. This questionnaire is a first start for us to get there, to better understand where you're coming from and be more clear how I can be of support. All work starts with a pre session where we get to know each other to understand if we can work together. Pre sessions take 30 minutes and cost 60,00€/55,00£. All other regular prices are found on the website.

Payments for sessions need to arrive on my account 24 hours before a session starts. Payment is via bank transfer. Cancellation and refund is only possible 24 hours before the session starts. By signing below you confirm that you have read and agree with these conditions.
