This prototype has two purposes. It demonstrates the user experience of this design, including how it changes for applicants with different circumstances. It shows the underlying branching logic – although complex – can be implemented.
As a prototype, this form has some differences from a recommended final design.
Its appearance does not match the good practice standards we recommend for public service forms, for example the
GOV.WALES or
GOV.UK Design Systems.
Questions in this prototype have been left optional to make it easier to review the form. This allows you to move quickly between pages without answering all the questions.
The exception is for questions where the form branches based on the answer. These are indicated with an asterisk. Please fill in these fields, using realistic dummy data if required, as the prototype will not behave as intended otherwise.
Questions should be mandatory in the real form, except those labelled 'optional'.
The prototype does not check whether a valid version of what the question has asked for has been entered, e.g. email addresses, phone numbers, post codes etc.
The prototype does crude calculations on dates provided by the applicant. It only uses the year entered and does not know today's date.
This affects: working out whether someone is pension or working age based on their date of birth; working out whether someone moved to their current address within the last 12 months. Using dates with a year comfortably within the range you want to test avoids any issues with this.
