UCF Adoption Fund Application

The first step to apply for a grant fund through Upward Christian Fellowship is by completing this online application. The Adoption Fund committee will prayerfully consider all applications. Each family is asked to understand that accepting the application is not a guarantee of receiving any kind of assistance.

How to Complete the Application

To complete this application, be prepared to input personal information for both parents, a financial breakdown of your adoption, details of the status of your adoption, and to submit the documents that are listed below under "Requirements". Please note that this application will save as you go, but will not save over multiple sessions. Both parents need to be present to complete the application and should allow about an hour to complete it.

Next Steps

Each application is structured to be assessed in the following areas: Heart for Adoption, Spiritual Wellness of Family, Educational and Parenting Values and Financial Stewardship.

If approved, you will be contacted via email with next step instructions. The Adoption Fund committee does not provide an explanation of approvals or denials. Please complete the application as thoroughly as possible to prevent delays in processing.

Requirements

As part of your completed application, the following documents are required. You will be prompted to upload these documents on the last page of this application.

- Copy of your completed Home Study.

- Copy of your current driver’s license for both applicants

- Copy of the first 2 pages of your most recent Federal Tax Return

- A current credit report card for you and your spouse (creditkarma.com or similar service that includes credit score and verification of total reportable debt.)

- A current check stub for both applicants (if applicable)

- A good quality photo of your current family

- Reference Letters: A reference letter from your Pastor, an employer/co-worker and one from someone not related to you (3 total). By checking this box, we as the adoptive parents agree that we can demonstrate:

A plan for a successful adoption, including evidence of the financial stewardship needed to complete the adoption, a community of support which includes local church membership, and a spiritual maturity to raise their child(ren) in a biblically-based home.
