When an insurer audited your notes or asked for money back

About 10 minutes. Please describe real events, not opinions. Short answers are fine, and "never happened" is a useful answer too.

I'm researching this problem to decide whether to build a product. Nothing is for sale here. I'm Hieu Tran; I use AI tools to help with this research, including drafting messages and organising responses. Answers are kept private and only shared in anonymised form.

Please don't include client names, dates of birth or anything from a client's record. Describe what the insurer asked for and what happened, not who it was about.

Think of the last time an insurer or its auditor asked for your records, sent a letter about how you bill (for example 90837), or asked for money back. Which payer or auditor, when, and what exactly did they ask for?

What did it cost or put at risk: the amount demanded, hours pulling charts, payments held back, a lawyer or consultant, sleep?

Walk through how you responded: what you sent, which deadlines you were given, who helped, and how it ended or where it stands now.

What have you used or paid for to prepare or respond (Headway, Alma or Grow support, your EHR's templates, an AI note tool, a billing service, an attorney, your professional association)? What was missing?
