By signing this form, you are agreeing you are the above player(s)/cheerleader(s)' parent/guardian. By signing you are agreeing that your child(ren) will be participating in the KYFL camp that will focus on conditioning and preparing the youth in our league for the season. You understand this is a two day event and that this is not a season sign up form.
In addition, signing below you are agreeing you will ABIDE by the parent/guardian code of conduct. This Code of Conduct must be signed and on file with the KYFL board prior to the player/cheerleader’s first game. By signing this document you are also agreeing that family, relatives and any other parent/guardian of the player(s) on this application will abide by all KYFL Parent/ Guardian Code of Conduct rules and regulations with the understanding that if this is not done, you and your player/cheerleader may be asked to withdraw from the league. This includes any KYFL By-Laws and KYFL Board rules that may or may not be on the Parent/Guardian Code of Conduct Agreement.
