What is Care Coordination?
Care coordination is a service that helps you navigate and access the support and resources you need for your health and well-being. A Care Coordinator works with you to connect you to the right services, providers, and community resources so you don't have to figure it all out on your own. Think of it as having someone in your corner to help you stay organized, informed, and on track with your care.
Please note that this form is only for adults ages 18+.
Please take 10 minutes to fill out this form. Please have the following ready to upload:
- Photo ID for proof of Illinois residency.
- Proof of low-income or public assistance
