Do not upload multiple swings, full rounds, extra time before or after the swing, or long untrimmed videos. 1. Performance & Liability Disclaimer

I understand that this is an AI-assisted swing analysis based on visible movement patterns only. This analysis is an educational tool and does not guarantee specific performance results. It does not replace in-person coaching, medical advice, or a professional physical evaluation. B3 Human Performance is not liable for any injuries, setbacks, or performance plateaus resulting from the implementation of this feedback.

2. Video Quality & Diagnostic Accuracy

I understand that the quality of the B3 Swing Diagnostic depends on the quality of the video submitted. B3 may request a new video if the footage is unclear, too long, trimmed incorrectly, blocked, or submitted in a way that does not follow the instructions provided.

3. Data Processing & Report Creation

By uploading this video and completing this form, I grant B3 Human Performance permission to review, store, process, and use the submitted video, form responses, and athlete performance data to produce an individual B3 diagnostic report, supported by B3’s AI-assisted swing analysis process.

I also understand that B3 may use submitted data internally for research, quality improvement, diagnostic system refinement, and AI-assisted model improvement. Submitted data will be handled in accordance with B3’s Privacy Policy.

4. Marketing & Public-Use Release

I grant B3 Human Performance permission to use the athlete’s name, image, likeness, voice, video, submitted footage, testimonial statements, and performance data for marketing, promotional, educational, and proof-of-results purposes.

This may include, but is not limited to:

social media
reels
stories
YouTube
email
advertisements
testimonials
case studies
sales materials
internal or external presentations

Performance data may include exit velocity, strength metrics, speed metrics, jump metrics, bodyweight, before-and-after results, testing dates, testing tools, athlete development outcomes, commitments, contracts, or other athletic achievements.

I understand that individual results vary and that no result shown or discussed by B3 guarantees future performance. I also understand that no compensation is owed for these uses.

I may withdraw consent for future marketing use by providing written notice to B3 Human Performance. I understand that materials already published, distributed, shared, or used in prior marketing may not be fully recoverable.

5. Content Ownership & Submission Rights

I confirm that I own the rights to the video being submitted or have received permission from the owner to submit it to B3 Human Performance. I agree not to submit footage that I do not have the legal right to share.

6. Age & Minor Consent

If the athlete is under 18 years old, I confirm that I am the athlete’s parent or legal guardian and that I have authority to submit this form, upload the video, and grant the permissions described in these terms on the athlete’s behalf.

If the athlete is 18 or older, I confirm that I am the athlete or have authorization from the athlete to submit this form and grant the permissions described in these terms.

7. Proprietary Access & No-Sharing Rule

This submission link and the resulting diagnostic reports are the intellectual property of B3NHP, INC. DBA B3 Human Performance.

Access is restricted to the intended user, athlete, parent, guardian, or active B3 Lab member. Sharing this link, redistributing reports, copying reports, or providing access to non-authorized individuals without B3’s permission is prohibited and may result in termination of access or membership without refund.
