BACKGROUND CHECK AND SIGNATURE

Because we support people with disabilities and other health needs, everyone who works with the people we support must pass background checks first.

If we offer you a role, the offer will depend on your background check results. The checks may include:

• Criminal history (state and national) • The NC Health Care Personnel Registry • The federal exclusion lists (OIG and SAM)Sex offender registries • Your driving record, if the role includes driving • Your licenses, certifications and training

We won't run any check until we give you a separate disclosure form and you sign it to give permission. That form also explains your rights under federal law.

Agreeing to a background check doesn't mean you have the role or that we'll make you an offer. A record doesn't automatically rule you out. We look at what happened, when it happened, and whether it relates to the role. Before we make a decision based on your report, we'll send you a copy and give you a chance to explain or correct it.
