This form covers all our policies, permissions, and important personal information. Please read through carefully and provide as much information as you can to help us support your child and make sure they have the best experience at camp!

I hereby give consent for my child to attend walking field trips to Palmer Park & Splash Pad, or to walk to a nearby park for outdoor play.

We are unable to keep children behind at the studio while the rest of the group goes outside. If a child is unwell and unable to join us, we will call home for a parent to pick up.

I would like to order a KAT t-shirt for my child to keep.

Our shirts are great as art-smocks, and we require each child to wear one whenever we leave the studio. Our studio will provide a borrowed shirt for any child that does not own their own. Please send t-shirt size in the description with your e-transfer to create@kidsarttalks.ca . The total including tax is $24.86. Shirts will only be ordered once payment is received.
