Investors, acquirers, and enterprise customers all check the same three things: whether your company is properly formed, whether the cap table matches reality, and whether the company owns what it sells. Those are solvable problems. Late and expensive, but solvable.

This questionnaire asks a different question about your technology: who made it, and can you prove it?

If the answer to this question isn't "yes," you may have a problem that isn't solvable late. If no person made a creative contribution and no record exists, there's nothing to assign, nothing to register, and nothing you can honestly warrant. No amount of cleanup in year four creates it after the fact.

If your team used generative tools to build product, write code, draft copy, or produce design assets, work through this before a program, investor, or enterprise customer works through it for you.

Answer honestly, including the questions you're unsure about. "Not sure" scores worse than "no" here — a known gap has a fix, an unknown one doesn't.
