Before completing this waiver, you must arrange your free trial or drop-in Taekwondo class directly with Master Kiani and receive confirmation of your class date. If she has not confirmed your date, please stop and contact her first. Submitting this waiver does not schedule, reserve, or guarantee a class. Do not attend without Master Kiani’s confirmation.
Aloha and welcome to Tropic Lightning Taekwondo Hawaiʻi! This form is for a free trial class or a drop-in class for visiting Taekwondo students. A parent or legal guardian may list up to three minor children; each participating adult must complete a separate waiver. Drop-in training is subject to availability and instructor approval. This waiver does not reserve a date, collect payment, or enroll anyone in the program. Enter only the class date Master Kiani confirmed for you.

Submissions are accessible to authorized school personnel and processed by our form provider. Relevant safety details may be shared with emergency responders when needed.
