To activate your device warranty, please complete the following form using the same information you used when placing your order.

* You can find your order number in the confirmation email

Add the serial number of each FlamAid you purchased.

*You can add up to 5 devices. If you purchased more than 5, please complete another form for the remaining devices.

1. Make sure your FlamAid is connected to the FlamAid app. Go to the Bluetooth section of the app.
2. You will find the serial number of your FlamAid device in the top-right corner of the app.
