DemBloc Governance Audit: Evidence submission form

Purpose. Use this form to submit documentary material relevant to an alleged governance infringement, implementation failure or structural weakness. Submission does not mean the audit group accepts the allegation or interpretation as fact.

Please do not upload safeguarding or disciplinary case material, passwords, raw membership data, or personal/sensitive information via this form. While submissions using this form are stripped of IP and other tracking data, please submit sensitive files via the encrypted audit email address provided by the audit team.
