This questionnaire collects the information needed to prepare your Lasting Power of Attorney documents.
You can use it to put in place:
- One Property and Financial Affairs LPA
- One Health and Welfare LPA
- Both types of LPA
- LPAs for two people completing the process together
Each LPA is a separate legal document. Where two people are completing this questionnaire, each person must provide and confirm their own instructions.
The questions will adapt according to the number and type of LPAs you select. You will be asked about the people you wish to appoint, how they should make decisions, any replacement attorneys, your certificate provider and any wishes or instructions you would like included.
You do not need to understand all the legal terminology before you begin. Explanations are provided throughout, and you can indicate where you would like further guidance.
