Lineup Chicago Applicant Questionnaire

The Lineup Dinner is a program designed to give cooks working on the line the guidance and support they need to confidently bring their vision to life. Guests will get to experience raw creative expression from the next generation of culinary talent.

This questionnaire helps us get to know you, your story, your inspirations, and how you approach your craft.

There are no right answers. We are simply looking to understand who you are, how you think, and how you work. Your responses will help us determine how and when we might collaborate.

The form includes six sections and should take 30 to 45 minutes to complete. You are welcome to take a break and return to it later if needed.

As a reminder, the final day to submit an application for our current round of casting is Monday, July 27th and we encourage you to submit before then if you can. Our team reviews applications in the order they are received.
