By signing this form, you are agreeing you are the above player(s)/cheerleader(s)' parent/guardian. By signing you are agreeing to pay the sum of $250.00(This price is for one uniform replacement, if signing up multiple children the price would be for each uniform not returned not just $250.00) for unreturned uniform items and any uniform items not turned in to KYFL in good condition. when asked to be returned in by the above player/cheerleader’s coach (This is done when a player/cheerleader is withdrawn or asked to leave by the KYFL Board, or at the conclusion of the last game/ Super Bowl).
In addition, signing below you are agreeing you will ABIDE by the parent/guardian code of conduct. This Code of Conduct must be signed and on file with the KYFL board prior to the player/cheerleader’s first game. By signing this document you are also agreeing that family, relatives and any other parent/guardian of the player(s) on this application will abide by all KYFL Parent/ Guardian Code of Conduct rules and regulations with the understanding that if this is not done, you and your player/cheerleader may be asked to withdraw from the league. This includes any KYFL By-Laws and KYFL Board rules that may or may not be on the Parent/Guardian Code of Conduct Agreement.
