New Client Intake Form

Please note that submitting this form *does not* guarantee an appointment. All forms are reviewed individually. I will reach out directly if I feel we are a good fit and would like to move forward with booking. Due to the volume of submissions, you may not receive a response if I am unable to accept your request.

Please upload clear photos of your curls dry *with* product applied. Include one front, one side, and one back view. Hair must be worn down and styled prior to any haircut appointment. No clips, buns, or ponytails should be worn prior. No oil or heavy butters. Please arrive with your hair styled as you would typically wear it before a night out - your best representation of how you like to wear your hair.
