OAKHAVEN RESCUE: Pig Adoption Application Form

Thank you for your interest in adopting a potbellied pig from Oakhaven Rescue!

This application helps us ensure the best possible placement for our pigs. All information will remain confidential, and the form takes about 10–15 minutes to complete.

As part of our adoption process, a mandatory home check is required before final approval. This may be completed either in person or via FaceTime or video call and helps us confirm a safe, appropriate environment for the pig.

Adoption Fees (please read before continuing):

• Male pigs: $500 • Female pigs: $1,000

Once your application is approved, a $100 non-refundable deposit is required to hold your pig until pickup. The remaining balance is due at the time of pickup.

After submission, we’ll review your application and reach out within 2–4 business days to discuss next steps.
