We have heard of patient safety concerns regarding doctors and want to learn more about the extent of these concerns to better understand the experiences of healthcare staff working with doctors, and of patients receiving care.

This form allows healthcare professionals and patients / relatives to provide details of specific incidences of harm, or any concerns they have about doctors.

Please use it to share your experiences without identifying any doctor or patient. Have you been mistreated, misdiagnosed, abused? Have you seen a doctor performing dangerously, working below the expected standard, or above their level of competence? Have they misdiagnosed you or a loved on? Been rude or dismissive?
