To register, simply fill out the below form and submit payment via Venmo or by sending a check. For multiple players, please submit the form for each player. Payment can be a single payment, though please note the player name(s) on the payment.

Please enroll the above signed. I understand that neither Nauset Regional High School nor anyone associated with the soccer camp will assume responsibility for an accident or medical expense incurred as a result of participation in this program. The applicant is in good health and is able to participate in physical activity of a vigorous program. In the event of injury, the soccer camp has my permission to PROVIDE/ACQUIRE MEDICAL CARE or ASSISTANCE.
Participants who are removed (behavior) from the program are not entitled to a refund.
