Starfest Music Festival Artist Application Form 2025

Thank you for applying to be an artist at the 2025 Starfest Music Festival! Please answer the following questions for your performance;

What is your artist name/stage name?

What is your performance setlist?

Please note that each performance set is limited to 15 minutes per artist. If your setlist is longer than 15 minutes, please shorten it. If any songs you are performing are covers, please add next to the song title the name of the artist/band that originally performed it.

What equipment/instrument will you need to bring for your performance? If none, you may skip this question. We will supply two microphones for vocals or mic'd instruments.
